In a typical onboarding flow:
- Tell FormIQ your practice details — region (AU/UK/US/CA/NZ), industry (medical / allied health / veterinary / legal / financial), and specialty (e.g. psychology, physiotherapy, podiatry).
- Optionally upload your existing practice documents (intake PDFs, consent forms, policies). FormIQ uses RAG to extract patterns specific to your practice.
- Click generate. You get a draft intake form covering the regulatory requirements for your jurisdiction — for Australia, that means AHPRA-aligned consent language, Privacy Act 1988 / Australian Privacy Principles disclosures, and Medicare item-relevant questions.
- Edit the form in the dashboard, then deploy with one click to Google Forms (with linked response sheet).
- Embed the form on your existing Wix site or share the Google Forms link directly.

Is FormIQ AHPRA-compliant out of the box? The generated forms include AHPRA-aligned consent language and the disclosure structure required by the Privacy Act 1988. AHPRA compliance ultimately depends on your practice’s specific scope and how you use the form — we recommend reviewing each generated form with your principal practitioner before deployment. FormIQ does the 80% you’d otherwise pay a consultant for.
Do I need a Wix site to use FormIQ? You install FormIQ from the Wix App Market, so a Wix account is required. The forms themselves can be embedded anywhere — on your Wix site, your existing WordPress practice site, or shared as a direct Google Forms link. We can help you migrate to or build a new Wix site if you don’t have one.
What happens to my patient data? Form responses go to a Google Sheet linked to the deployed Google Form, in a Google account you control. FormIQ never sees response data — it only generates the form structure. For practices that need stricter data residency (Australian-only hosting), pair FormIQ with Cloud Geeks managed cloud.
How does FormIQ compare to a generic Google Forms template? Generic templates aren’t specialty-aware (the AHPRA requirements for psychology differ from physiotherapy from podiatry) and don’t reflect your practice’s existing policies. FormIQ does both, in minutes, with optional document context.
